JOB DESCRIPTION JOB TITLE : Case Manager JOB CLASSIFICATION : Non-Exempt Full-Time DEPARTMENT : Catholic Social Services, Women of Hope-Vine Housing & Homeless Services Division REPORTS TO : Social Work Supervisor DIRECT REPORTS : NONE MAJOR RESPONSIBILITIES : Provide intensive case management services to assigned residents at Women of Hope-Vine. Protects clients' rights to privacy in a manner consistent with confidentiality guidelines and professional standards of care and sensitivity. Gain a thorough understanding of protocols to ensure that all contractual obligations with the Department of Behavior Health and Intellectual Disability Services (DBHIDS) and PA Department of Human Services, as required for Licensed Personal Care Homes (PCH) are met. Provide direct support for residents with their mental, medical, physical, and emotional needs for them to move towards full independence. Collaborate with all connected client resources/ interdisciplinary services and extended family to provide needed support and advocacy for the resident. PRINCIPAL DUTIES AND RESPONSIBILITIES: Provides comprehensive intake assessment, reassessment, service planning and discharge planning of each assigned resident. Assist residents with moving into facility; manage residents' orientation and curfew. Manage residents' medications, log in all incoming meds, complete monthly med sheets in Harmony, and provide med education. Monitor amount of meds resident has, including refills. Assist residents in obtaining refills when necessary. Pick up medication from pharmacy. Regularly (weekly or bi-weekly) meet with assigned residents. Works with residents to establish realistic goals and monitors' progress; collaborates with ICM's, R.C.'s, etc. for effective coordination of these goals. Attend some and follow up on all physical and mental health services necessary and which are required to be in full compliance with the state's PCH regulations. Maintain accurate, up to date records, progress, and medical notes for assigned residents. Assists residents in exploring and applying for appropriate entitlements. Provides ADL and Personal Care Services as needed including financial management. Investigates possible programs (job training, social rehab, dual diagnosis group meetings etc.) in preparation for eventual transition to permanent housing. Meets with social work supervisor weekly, both individually and as a team to discuss progress of residents and to seek advice as well as to share ideas. Maintain open communication with Housekeeping/Maintenance, Resident Aides, and Kitchen staff as it pertains to resident needs and restrictions. Share On-Call responsibilities with case management team monthly. Work as a team and assist in other areas when needed, including the kitchen, filling in for other case managers, etc. Flexibility in work schedule. Available to assist in recreational events, shopping with residents and planning holiday activities. Attends yearly training in regard to CSS, DBHIDS and PA DHS policies. Attends staff meetings and makes use of in-service opportunities. Assists social work supervisor and/or program director in any assignment deemed necessary for the welfare of the resident and the house. Other duties as needed. REPRESENTATIVE KNOWLEDGE, SKILLS, AND ABILITIES Knowledge of, and orientation of philosophy of the Agency as presented in Mission/Philosophy Statements Bachelor's degree in social work Strong organizational skills and ability to manage multiple tasks and respond to crisis with appropriate interventions. A desire, interest, and ability to work with women with severe and persistent mental health disorders who have experienced homelessness. Works as a team member, exhibits respect, hospitality and always maintains professionalism. Have or will obtain a current certification in CPR and First Aid within 30 days of hire. Has or will completed the "Direct Care Staff Person Training Course/Test online within 30 days of hire and submit the training certificate. Has or will complete the state's medication administration training along with additional Women of Hope Med Training to be qualified to distribute medications within 30 days of hire.
