A company is looking for a Utilization Management Social Worker in a remote position. Key Responsibilities: Conduct telephonic social work outreach to assess behavior health issues, caregiver support, and placement barriers Document social work interventions and plan of care in the health plan's UM platform Provide appropriate interventions to optimize health and well-being, including care coordination Required Qualifications: Unrestricted California Social Work license (LCSW) or Master's degree in Social Work (MSW) Minimum 3 years' experience in care management with geriatric population Experience with motivational interviewing Background in palliative care and hospice programs preferred Spanish speaking skills preferred
