This position is a great opportunity to grow your RN skillset while providing essential one-on-one care to individuals in the home care setting. Responsibilities * Implement the treatment and rehabilitative strategies outlined in the patient care plan, including medication management, rehabilitation treatments, coordination of medical needs, and budget monitoring--- Maintain thorough and timely documentation of care provided to ensure accurate patient record and billing--- Conduct assessments and approve plan of care and service delivery methods--- Follow consumer health plans, provide and promote consumer choice, and facilitate consumer independence--- Perform competency and skills evaluations on employees who provide direct care--- Other tasks as assigned Qualifications * Must be at least 21 years of age--- Active Registered Nurse (RN) license in good standing in state of operation--- Bachelor's Degree in nursing or related health and human services field from an accredited college or university OR an Associates Degree in nursing from an accredited two-year program About our Line of Business At Revival Home Health & Hospice, our primary ambition is to bring first-class care to the warmest and most serene environment possible: home.
