Our services are spread out nationally so there will be travel required. Here at Home Group clinical intervention work goes hand in hand with day-to-day support. We’re trailblazing the way in delivering brilliant long-term outcomes for vulnerable people through integrating health, housing, and care. You will work as part of the central clinical hub using your clinical expertise to provide support, guidance and advice to our colleagues across the country who work with our customers. You will enable us to maintain high standards of support, embed the principles of Psychologically Informed Environments, support teams to better understand the complexities of customers, and contribute to achieving brilliant outcomes. We aim to support our customers to meet their aspirations and to work towards living independently and feeling enabled. As a Mental Health Practice Clinican you will: Support and develop our team to maximise customer independence and improve outcomes. Support the identification and management of risk in order to achieve best outcomes for customers. Support and facilitate practice development in order to enhance our colleague teams within services across the country to directly support our customers. Promote regular, honest, and supportive feedback through reflective practice and group supervision. Enhance support colleague engagement with external professionals and providers to ensure our customers receive the support, they require through a multi professional approach. Have the flexibility.
