* The Clinical Social Worker is a grant funded position; therefore, the duration of this position is subject to the guidelines outlined in the grant. SUMMARY The Clinical Social Worker will provide confidential service to clients requiring case management, prevention and intervention services to Adult Serious Mental Illness (SMI) and Elderly Dementia clientele that may also possess Co-occurring Disorders (COD). The Clinical Social Worker will be responsible for working collaboratively with multiple programs and agencies to ensure the highest level of care and access to services for their clientele. This position will be responsible for maintaining data that captures qualitative and quantitative information for the client's health record. Grand Traverse Band is an NHSC Approved Site (GTB Clinic, GTB Dental and GTB Behavioral Health) *Licensed primary care clinicians in eligible disciplines can receive loan repayment assistance through the NHSC Loan Repayment Program (NHSC LRP). You must serve at least two years of service at an NHSC -approved site in a Health Professional Shortage Area (HPSA) such as Federal Indian Health Service (IHS) Facility . MINIMUM QUALIFICATIONS Bachelor's Degree in Social Work State of Michigan Social Work License Must have knowledge of records management Must have a valid unrestricted driver license and be insurable by the GTB insurance carrier ESSENTIAL DUTIES AND RESPONSIBILITIES Coordinate services with other Tribal programs and/or County and State agencies such as: Community Mental Health (CMH), Department of Health & Human Services (DHHS), mental health counseling, substance abuse treatment, and other related social services. Serve as a client advocate collaborating with courts, hospitals, nursing homes, community resources, police and other Tribal services. Develop treatment and case management plans in conjunction with clients input, team case consultation and the GTB Contractual Psychiatrist. Assess the needs of the client with family input where appropriate, in conjunction with psychiatrist and psychotherapists, participate in developing goals and objectives integrating strong support plans, and aftercare treatment plans Locating appropriate resources and assisting with appropriate referrals. Maintain regular contact with assigned clients in accordance with the program and grant guidelines. Assure the client's safety and well-being as first-priority for mental wellness and living environments. Maintain patient confidentiality as per Health Insurance Portability and Accountability Act (HIPAA) of 1996 and complete HIPAA compliance training. Accommodate schedule as necessary in emergent client situations along with assistance for transporting by medical care staff when applicable. Participate in required case-management related activities (e.g. court hearings, pre-hearing conferences, staff meetings, case conferences). Attend staff/team meetings, case conferences, in-service trainings, etc. to share knowledge and acquire ongoing expertise in case management promoting optimum care. Keeps statistical data and assist in completion of monthly, quarterly, year-end budget and grant narrative reports. Provide education to the community and other GTB Departments on SMI diagnosis, dementia, and other Co-occurring Disorders. Assist clients in petitioning for Guardian and Conservatorships Documents and maintain clinical records in accordance with Tribal, State and Federal regulations. Pursue and attend trainings as required or necessary (i.e. evidenced based therapeutic models and trainings provided by other agencies on veteran's benefits, health, social services, retirement programs, etc. Other duties as assigned OTHER SKILLS AND ABILITIES Knowledge of and adherence to the highest level of confidentiality on all matters involving clients and families being served or visiting the AFS and BHS Offices, as outlined in Federal Law and Regulations 42 CFR, Part 2, as well as those outlined in the GTB Personnel Policy booklet. Knowledge of and adherence to State Licensing regulations and requirements. Must have outstanding written and verbal communication skills Good time management and organizational skill are critical Excellent interpersonal skills with demonstrated seven grandfather teachings. Uses judgment in identifying and selecting the most appropriate procedures to use in determining established alternatives in providing care Utilizes established procedures and specific guidelines available through resources including, but not limited to procedural manuals, textbooks, and work samples Collaboration with clinical staff on complex cases. Knowledge of medications, applicable law, regulations, rules, procedures, and administration is required Exceptional detail and follow-up skills Effectively establishes priorities and meets deadlines Ability to quickly evaluate alternatives and decide on a plan of action Ability to effectively establish priorities and meet deadlines. Ability to exercise considerable discretion in handling delegated administrative details and various clerical duties with minimal procedures and systems for the efficient and accountable operation of the program. Proficiency in computers using Microsoft® Windows Operating System and Office Suite and use of other office equipment. General knowledge of applicable law, regulations, rules, procedures, and administration is required as it pertains to position. EDUCATION and/or EXPERIENCE Bachelor's Degree in Social Work required. State of Michigan Social Work License. 2 years of case management experience preferred. Successful completion of Program Director Training within one year from the date of hire. MAFE MCBAP or extensive knowledge of chemical dependency issues. Must have knowledge of clinical records management. DRIVING REQUIREMENTS Must have a valid unrestricted driver license and be insurable by the GTB insurance carrier. SUPERVISORY RESPONSIBILITIES No supervisory responsibilities required EQUIPMENT TO BE USED General office equipment such as calculators, computers, copiers, fax, phone system, and so on TYPICAL PHYSICAL DEMANDS Involves sedentary work, with some lifting, reaching and walking and lifting heavy objects, such as a case of paper or several books at once. Also requires manual dexterity to operate office equipment, keyboarding, and so on. TYPICAL MENTAL DEMANDS The complexity and responsibility inherent in this position requires the candidate to be able to perform independently, calmly and decisively with minimal supervision, in response to the demands of the position and with above average ability. Deadlines are involved that may produce stressful conditions on occasion. WORKING CONDITIONS Normal office environment with anticipated automobile travel for client visits in all six-county service area. Some work to be performed in the three GTB satellite offices (Traverse City, Charlevoix, and Benzie). COMMENTS Native American preference will apply. Must be willing and able to pass a background investigation and a drug and alcohol urinalysis as a condition of employment. Adherence to strict company policy regarding confidentiality is a must.
