Multicultural Community Services is seeking a compassionate RN to join our AFC team to provide nursing care to client's in a family setting. Adult Foster Care (also known as AFC), is a program for Elderly adults or adults with developmental or physical disabilities who cannot live alone safely. AFC Members live with trained, paid Caregivers who provide daily care. On a daily basis, AFC provides individuals with personal care assistance in a family setting rather than a nursing home or other residential facility. Caregivers provide meals, companionship, personal care assistance and 24-hr supervision. Caregivers may be individuals, couples or large families. As an AFC Registered Nurse, you will be responsible for providing exceptional nursing care and services to individuals residing in the community with a Caregiver. Your role will encompass a wide range of tasks, including assessing patients' health conditions, administering medication, collaborating with healthcare professionals, and ensuring a safe and comfortable environment for the individuals.Some responsibilities will include: Facilitate quality services through adherence to agency and AFC standards. Adhere to the Adult Protective Services and Disabled Persons Protection Commission’s Mandated Reporter Guidelines. Complete and sign the designed screening tool as required for Mass Health Payment. Complete the MDS-HC or CDS and other nursing assessments and coordinate all other applicable clinical assessments. Develop and review on an ongoing basis each member’s AFC plan of care. Select, train, evaluate and supervise AFC caregivers in conjunction with the Care Manager. Responsible for reviewing the physician summary. Monitor the health status of all members to ensure that all needed AFC care is properly delivered. Report changes in the health status of any member to the member’s physician. Periodic review AFC caregiver logs. Conduct on-site home visits with each member at the qualified setting as required by regulation. Complete nursing progress notes. Plan and implement discharges from AFC program. Conduct an orientation for each AFC caregiver before the AFC caregiver begins personal care. Represent MCS as requested within the community and with other agencies / organizations with similar objectives. Participate in conferences, workshops and related trainings to stay abreast of developments in the field. Provide training to AFC caregivers. Submit a semiannual health-status report to the member’s primary care physician. Practice HIPPA Privacy Rules and the agency’s health information policies and procedures. QUALIFICATIONS: The Registered Nurse must be fully licensed by the Massachusetts Board of Registration in Nursing and a minimum of two years of recent professional health-care experience working with elderly or disabled adults. RN must also have a valid driver’s license and access to an automobile.
